You are about to be discharged from hospital

What the Council can do

When you ask the Council for assistance you should be ready to give as many details as possible, including:

  • contact details for your hospital discharge team
  • reasons you cannot return to where you lived before you went into hospital
  • identification - for example, a copy of your passport, birth certificate or Home Office papers
  • your financial information - for example, bank statements and benefit letters

The Council may:

  • work with the hospital discharge team
  • help you identify any places where you have a right to live
  • put you in touch with other organisations that could help you
